Ofsted inspectors are reportedly fabricating evidence due to frequent crashes in the electronic evidence gathering (EEG) system introduced in 2018. The Observer's investigation reveals anonymous claims that senior Ofsted leaders have known and covered these technical problems. Critics argue that inspections over the past five years should be invalidated if widespread evidence fabrication is true. Ofsted insists that judgments are backed by sufficient evidence, but the Observer found evidence of potential issues in Ofsted's written response to a school's complaint about missing evidence.

According to the Quality Assurance Agency for Higher Education, there are over 1,000 essay mills in operation in UK universities. The government announced that offering essay-writing services to students for a fee will become a criminal offence under plans to tackle cheating. Universities welcomed this decision and introduced codes of conduct with severe penalties for submitting work that is not a student's own.

The Ministry of Education has made a complaint against three to four institutions in Ile-de-France regarding questions leaked. The 7,500 academies in England educate nearly 4 million pupils. Thousands of pupils are being failed by a system that fails to give academy trusts proper oversight. The public accounts committee reported that governance of academy trusts must be strengthened and that the Department for Education’s oversight must be more rigorous. MPs have called for new sanctions to deter and punish malpractice after hearing that the former head of Durand Academy in south London was paid an £850,000 severance package.

According to Slovak media the Speaker of the Slovakian Parliament has been accused of having copied his JuDR doctoral thesis in law from five other sources. The politician put his thesis in the university library under embargo, when the accusations first arose. Comenius University announced that a doctoral dissertation with the same title and same number of pages was missing in its university archive and that an enquiry has been launched.
